    This is probably so late, like “so 2000-and-eight,” but I am just so giddy about the thought that I can MAKE MY OWN- as in PERSONALIZE MY ICONssss! (I made the one that says, “CHAPTER 3.”)
Open WORD and make whatever design, paste picture, letters, etc., on the blank page.
Save it as PDF.
Then save it again from PDF to JPEG.
Select the part of the JPEG file that you wish to be your icon, then COPY.
Go to folder you wish to change into new icon, don’t open, just select then apple + i. 
Once that opens, go to the upper left-most corner where there’s a small icon of that folder, then click paste. That should change immediately it to your new icon.
